2016 was the deadliest year ever for migrants on the Mediterranean. Around 5,000 people died trying to make the crossing to Europe.

BBC Radio 5 live's Lucy Grey reflects on the 12 days she spent broadcasting live from a search and rescue ship as it pulled nearly 400 people from the sea near the Libyan coast. She hears harrowing descriptions of rape and brutality, families separated by gunfire as they try to flee the people-smugglers, and hears the story of a heavily pregnant mother who set out on this incredibly risky journey.
